Enrollment Documents
Families may print each PDF enrollment document and fill them out. can either email the completed documents or deliver them in person to Kumu Leti. The child's pediatrician must complete the 2 medical documents. Additionally, please include the child's birth certificate, immunization record and TB clearance.
